Case Management Services
Healthcare Commons Inc. offers a
full spectrum on Case Management Services for Adults, Children, and Families.
All Case Management Programs are community based services designed to assist
clients and families in need of mental health services. Each program has
a unique target population designed to serve those individuals most in
need of services.
Youth Case
Management (YCM)
The Youth Case Management Program provides comprehensive community-
based services to seriously emotionally disturbed youth who are under 18
years of age and their families. YCM services include, but are not limited
to:

Providing case and systems advocacy, service linkages and referrals
The intent of the YCM Program is to maximize each youth's potential for
learning, growth, and emotional stability within the family or natural
support system. This is achieved through the following goals:

Respecting the rights and dignity of youth and family members.

Helping youth and family members or legal guardians learn to manage the
youth's illness in order to prevent a relapse, re- hospitalization, or
placement in a more restrictive environment.

Empowering youth and their families to actively participate in treatment
and programming in order to determine personal and program goals.

Helping youth and their families develop and use social support systems.
-
Advocating for youths on their behalf.

Affirming each youth's strengths and abilities.
Intensive Case
Management (ICM)
Intensive Case Management shall provide services to persons 18 years
of age and older who have had difficulty maintaining stability in the community
with only traditional community mental health services. The goal of ICM
is to prevent further admissions to psychiatric facilities.
To meet the needs of these clients, ICM is designed to provide intensive,
primarily off-site community-based services and outreach affording the
client utilization of all available resources to address both basic and
unique client needs. This includes services not only to the client but
also to his/her natural support system (i.e. family, friends, employers,
and self-help groups) to the extent the client permits this. Clients enrolled
in ICM are eligible for services for a minimum of 18 months.
Homeless Mentally
Ill Program (HMI)
The Homeless Mentally Ill Program provides intensive outreach, evaluation,
referral, case management and treatment services to those homeless persons
who may be mentally ill.
This is accomplished through several points of emphasis:

The availability of a full-time HMI Case-manager position to serve this
population exclusively. In the absence of this individual (i.e. vacations)
coverage is provided and ensured through the use of Emergency Screeners
and/or other Case Management staff on a temporary basis.

The provision of effective outreach services to the homeless mentally ill,
including those staying in shelter situations within Salem County.

A combination of extensive and ongoing training of the case manager and
the establishment of effective linkages with other entitlement and social
service agencies is imperative, including but not limited to the County
Board of Social Services, the Inter-Agency Council, United Fund Agencies,
Social Security Administration, Medicare and Medicaid programs, transitional
housing programs, and utilization of referrals to other such service components.

Transportation assistance for clients involved in HMI
Services are available 24 hours/day, 7 days/week either provided directly
by the HMI Case Manager or through the Emergency Services Department of
the Agency when the HMI Case Manager is not available. Service after normal
business hours is achieved through an answering service and "beeper" service.
There are no fees to clients associated with the HMI program.
