Integrated Case Management Services (ICMS)
Providing ongoing community care to consumers at risk for psychiatric hospitalization

Intensive Case Management Services provides services to persons 18 years of age and older who have had difficulty maintaining stability in the community with only traditional community mental health services. 

The goal of ICMS is to prevent further admissions to psychiatric facilities. To meet the needs of these clients, ICMS is designed to provide intensive, primarily off-site community-based services and outreach affording the client utilization of all available resources to address both basic and unique client needs. This includes services not only to the client but also to his/her natural support system (i.e. family, friends, employers, and self-help groups) to the extent the client permits.
